Paying for Care

Thrive Alabama serves anyone who comes to us for care, regardless of their ability to pay or their insurance status. We believe that cost should not prevent you from getting the care you need. If you’re sick or it has been some time since you have had a primary care provider, call 256-536-4700 and make an appointment.

Your costs will vary based on the services provided, your insurance, your income, the composition of your household, and other factors. Thrive Alabama offers a sliding scale fee discount program on our services to people who qualify.

Using Insurance

Do I have to pay a copay when I come?

This depends on your insurance plan. If you have insurance, all applicable co-payments and deductibles must be paid at the time of service. We understand how confusing insurance can be and we will help you figure it out.

You may be eligible for the sliding fee discounts even if you have insurance. Please note, if you have more than one appointment in the same day, you may have two separate copays.

What if my insurance doesn’t cover everything?
Please be aware that some of the services you receive may not be covered – or not considered reasonable or necessary – by your insurance. You must pay for these services at the time of your visit, unless other arrangements have been made.
What if I don't want to use my insurance?
If you do not want to bill your insurance for services we can still see you. You will be considered self-pay and are responsible for your charges.
How do I know if the services are covered before I receive them?
Our goal is to ensure we have good communication with you about the services we provide to you. Our highly trained medical professionals determine what is medically necessary and reasonable. If there are questions, our staff will discuss this with you. Your insurance company may need to be contacted to see if services are covered. Please talk to our staff for more information.
What if I don’t want to accept a service unless it is covered by my insurance?
You can always opt out of treatment. If you are concerned about a service not being covered by your insurance please talk to your provider and our staff.
What about lab work?
If you have insurance, your labs will be billed directly to your insurance plan by the lab company. The lab company will bill you directly for any costs your insurance does not cover.
What are other arrangements?
We can work with you on a payment plan. Please ask our staff for more information.

Using Sliding Fee Scale Discount

How does the sliding fee discount work?

If you qualify for the sliding fee discount program, the lower your household income, the less you will pay towards your care. There are different levels. For example:

  1. If you have two people in your household and your combined household income is $16,460 you will be responsible for the nominal charge of $20 at your medical visit.
  2. If you have 3 people in your household, and your household income is $31,650 you will receive a 50% discount at your visit, and you will be responsible for 50% of the cost of your medical visit.
  3. If you have 1 person in your household and your income is $24,280 a year you will receive a 25% discount on the cost of your medical visit.
Who qualifies?
You may qualify for a discount if your family/household income is at or below 200% of the federal government’s Poverty Guidelines.
What do we mean by family or household?
We define family or household as individuals who share living arrangements, share expenses and income, or someone you are a caregiver for.
What are the poverty guidelines?
Every year the Federal Department of Health and Human Services (HHS) in Washington D.C. releases the poverty guidelines for administrative purposes — for instance, to determine financial eligibility to qualify patients to be eligible to pay reduced costs for services.
What do I need to bring in to qualify for the sliding fee scale program?
Please bring proof of income (2 current pay stubs, W2 , SSI, etc) for each member of your household with you to your appointment.
Do all services at Thrive Alabama qualify for the sliding fee scale?
Thrive Alabama offers all medical services on a sliding fee scale.
Are there options to get insurance?
Call 256-536-4700 to make an appointment. We can help you enroll in Medicaid, Medicare, or for the Affordable Care Act (during an open enrollment period or in the event of a qualifying life-changing situation) for health insurance coverage.
What about lab work?

The lab company will bill you directly.
